The main aim of this study is to explore the breadth of the inference field and the type of etiopathogenetic contents of symptom explanations provided by the client and therapist in the first two psychotherapy sessions conducted using a systemic approach. Does the therapist use triadic explanations of psychopathology as suggested by her approach? And do clients resort almost exclusively to monadic and dyadic explanations as did the university students in our previous study? What kind of explanations do they propose? (...) The coding system “1 to 3: from the monad to the triad” was applied to the transcripts of 25 individual systemic therapies conducted by the same therapist. This manual allows to code the inference field of symptom explanations according to three categories: monadic, dyadic and triadic. These three broad categories are also used to analyse the etiopathogenetic content of each explanation: traumatic, intrapersonal, and interpersonal. Our findings showed that clients and their therapist actually used different inference fields: clients resorted almost exclusively to monadic and dyadic explanations, whereas their therapist included the triadic explanatory level. Moreover, the therapist provided more interpersonal explanations than her clients. Hence, the dissonance between client and therapist about the inference fields¬–a crucial premise of one of the most accepted ideas of therapeutic change according to systemic therapies–is proved, at least among our participants. Thanks to this dissonance, clients and therapists can create a new story, potentially able to change clients’ feelings, without disconfirming their emotions. (shrink)

Drawing primarily from the cultural traditions and beliefs of the Muscogee peoples, I will provide an account of how harmony can play a foundational role in providing a structure to morality. In the process of providing this account, I will begin (§2) by defining two key Muscogee concepts: ‘energy’ (§2.1) and ‘harmony’ (§2.2). I will also explain how the relationship between these two concepts can provide a structure for morality. Then I will explain the conditions that make promoting harmony a (...) normative principle (§3) by explaining why promoting harmony is relevant to humans (§3.1) as well as a providing a prudential reason to promote harmony (§3.2). Finally, I will explain how harmony can be achieved (§4) by explaining two examples that highlight the importance of non-moral knowledge in promoting harmony. I will then conclude with some remarks about how the Muscogee concept of harmony relates to some contemporary metaethical concerns (§5). (shrink)

Quantifiers are a test case for an interface between psychological questions, which attempt to specify the numerical content that supports the semantics of quantifiers, and linguistic questions, which uncover the range of possible quantifier meanings allowable within the constraints of the syntax. Here we explore the development of comprehension of most in English, of particular interest as it calls on precise numerical content that, in adults, requires an understanding of large exact numerosities (e.g., 23 blue dots and 17 yellow is (...) an instance of “most of the dots are blue”). In a sample of 100 children 2 to 5 years of age we find that (a) successful most comprehension in cases with two salient subsets is achieved at 3 years, 7 months of age, and (b) most comprehension is independent of knowledge of large exact number words; that is, knowledge of large exact number words is neither necessary, as evidenced by children who understand “most” but not “four,” nor sufficient, as evidenced by children who understand “nine” but not “most.”. This essay makes the affirmative case for health reform by expounding on three fundamental points: one moral case for expanding access to coverage and care to all is grounded in scriptural concepts of community and mutual obligation which continue to inform the American pursuit of justice; the structure of PPACA springs from an appreciation of and approach to channeling market forces that was developed and proposed by a coalition of moderate and conservative Republican U.S. senators almost 20 years ago; the (...) most humane path to a better and more sustainable health system lies in implementing PPACA as fast and fully as we can. The purpose of this essay is to articulate why it is not possible to make our health system better, sustainable and serve us all without government playing specific and limited but absolutely crucial catalytic roles. It has become fashionable to question attempts to derive internationally agreed duties of clinical care from more general theories of human rights. For example, some argue that such attempts risk moral abstraction through their neglect for the importance of culture and community in shaping moral consciousness and are thus often unhelpful in the resolution of concrete moral dilemmas within medicine. Others denounce the importance of general moral principles altogether in bioethics and attempt to articulate what are claimed to be more (...) practical approaches to resolving moral conflict. This paper challenges such arguments. It does so through arguing that: i) all humans everywhere have the same basic human needs; ii) the satisfaction of these needs varies with culture; iii) the imputation of moral duties on others entails respect for their right to basic need satisfaction, including the right to choose between cultures; iv) internationally accepted clinical duties of care embrace presumptions about the duties and rights of patients which follow from these more general principles and v) problems of moral indeterminacy that arise from putting these principles into practice can be resolved through associated procedural policies of rational negotiation and compromise. The moral importance and practicality of respect for individual human rights within the practice of medicine is thus defended. Indeed, the paper concludes by arguing that without belief in human rights linked to a theory of basic human needs, communitarian theories of morality are incoherent. (shrink)

A programme of research into conflict (e.g. violence, absconding, medication refusal) and containment (e.g. seclusion, special observation, physical restraint) in inpatient psychiatry has been under way at City University, London, UK, for the past 10 years. Recent research findings, plus the challenges posed by ongoing projects, have made apparent the need for greater clarity about the overarching concepts of ‘conflict’ and ‘containment’. This paper pulls together research findings pertaining to this issue, and conducts a reasoned analysis of what common characteristics (...) might underlie ‘conflict’ and ‘containment’. It is concluded that these are patient threats to safety, and the staff maintenance of safety. Details are presented on the inclusions and exclusions that follow from taking such a position, and potential definitions offered. On the grounds of this conceptual analysis, plus evidence for moderate degrees of statistical association between behaviours and events in each domain, it is concluded that it is legitimate to conduct analyses at the level of total conflict and containment rates, as well as at the level of individual types of behaviours and events (e.g. verbal abuse, sedation). Some of the mathematical difficulties in the analysis of total conflict and containment are addressed, and results of a weighting exercise presented. This exercise challenges our perception of the severity of some containment measures that are becoming more commonly used in acute psychiatry. (shrink)

Income distribution continues to be a major problem of the countries. Turkey is one of the countries that are in the high Gini Coefficient if we compare it with the EU members. It is known that Turkey has applied various economic policies to solve this problem. In order to ensure social justice the application of redistribution policies is the first solution that comes in mind. However, its implementation option is limited due to its negative effect on economic growth. The most (...) effective option to ensure social justice is to give importance to the education sector without compromising on the economic growth. In this study we will discuss, firstly, income distribution movements over the years and analyze its impact on the political economy. Especially in the time period of 1977-2014, the effects of an increase in the economic growth due to an increase in the number of primary, secondary and university graduates will be investigated by using the ARDL boundary test approach. The obtained finding of the long term relationship of these variables and the contribution of their relationship on the estimated result of an increase in the education index caused by a decrease in the Gini Coefficient that is shown in the form of improvement effects of income distribution. According to the findings in the literature the investments made for education in Turkey have made a significant contribution to the improvement of income distribution. Lastly, in order to increase the influence of education on economy some policy recommendations have been discussed in the study. (shrink)

There is a respectable feminist tradition in utopian thought. Dreams and fantasies about gender-equal, women-friendly or female-dominated worlds have been formulated abundantly. However, utopian thinking has also met with severe criticism. By definition, utopias were said to be too idealistic, and of little use in the process of societal change. More recently, it has been stressed that the concept of utopia has been superseded by postmodern awareness, in which general explanations of gender inequality (and, along with them, general utopian views) (...) are disqualified to the benefit of more local and more specific theories. In this book, the reader will find not one general, broadly defined utopia, but instead, a wide array of more or less specific, feminist utopias. Utopias are viewed as preliminary and imaginary goals from which present situations can be revalued and from which strategies for change can be developed. Given that most faculty and administrators at these universities are laypeople and many are not Catholic, the universities depend largely on their campus mission/ministry offices for this purpose. (...) One of the primary methods of sharing their worldview has been to provide retreats for students.The retreats are introspective with a religious and Jesuit orientation. Most student retreats are conducted over a weekend; some over a full week. Most Jesuit universities get fewer than ten percent of their students to attend one retreat in their four years on campus. Far fewer students attend more than one retreat. These schools also attempt to encourage faculty and staff to attend similar retreats, and participation in these retreats is similarly modest.Retreats have been a primary tool for the formation of Jesuit priests. Their formation program includes taking at least two retreats of twenty-eight to thirty days each. These retreats use the Spiritual Exercises of Ignatius of Loyola, a specific pattern of reflective meditation and introspection over twenty-eight days that leads to greater clarity about one’s life. The Spiritual Exercises are deeply theistic and are focused on the person of Jesus.Jesuit universities have an increasing number of non-Catholics and non-Christians. The intensity of the opposition to health reform in the United States continues to shock and perplex proponents of the Patient Protection and Affordable Care Act. The emotion and the apocalyptic rhetoric, render civil and evidence-based debate over the implications and alternatives to specific provisions in the law difficult if not problematic. The public debate has largely barreled down two non-parallel yet non-intersecting paths: opponents focus on their fear of government expansion in the future if PPACA is implemented now, while (...) proponents focus on the urgency and specifics of our health care market problems and the limited number of tools we have to address them. Frustration on both sides has led opponents to deny the seriousness of our health system’s problems and proponents to ignore the risk of governmental overreach. These non-intersecting lines of argument are not moving us closer to a desired and necessary resolution. (shrink)

Drug recalls and lawsuits against pharmaceutical manufacturers are accompanied by announcements emphasizing harmful drug side-effects. Those with elevated health anxiety may be more reactive to such announcements. We evaluated whether health anxiety and financial incentives affect subjective symptom endorsement, and objective outcomes of cognitive and physiological functioning during a mock drug recall. Hundred and sixty-one participants reported use of over-the-counter pain medications and presented with a fictitious medication recall via a mock Food and Drug Administration website. The opportunity to join (...) a class-action lawsuit was manipulated. We assessed health anxiety, recalled drug usage, blood pressure, heart rate, and performance on a computerized Trail Making Test. Symptom endorsement was strongly predicted by health anxiety. When combined, three health anxiety measures explained 28.5% variance. These effects remain strong after controlling for depression and anxiety. Litigation condition did not predict symptom endorsement. Blood pressure and heart rate were modestly predicted by health anxiety, but not by litigation condition. TMT performance was consistently predicted by health anxiety, with higher scores associated with poorer performance. Although there were no main effects for litigation, interactions consistently emerged for the TMT, with generally poorer performance for those with higher health anxiety in the non-litigation condition; whereas health anxiety was unrelated to performance for the litigation condition. All but one participant joined the litigation when given the opportunity, despite a healthy sample and minimal use of pain medication. Subsequent data from 67 individuals with no mention of the FDA scenario or litigation showed that health anxiety still significantly predicts symptom endorsement, but the explained variance is less than half that obtained in the FDA scenario. The findings suggest that health anxiety plays a significant role in adverse symptom reporting, beyond anxiety or depression, and this effect is independent of the presence of the FDA recall. The lack of differences for health anxiety and symptom endorsement between litigation and non-litigation conditions rules out malingering. Although it is general practice in drug recalls to list potential adverse side effects caused by medications, this may elicit unintended symptom experiences and health anxious individuals may be more susceptible. To answer this question I turn to the work of two theorists: Luce Irigaray and Judith Butler. After an account of their ideas, various sexual practices and identities will be described. The question is whether these practices and identities affect the meanings of masculinity and femininity. It will be concluded (...) that, even in our postmodern world with its technological opportunities the division into the two sexes is extraordinarily persistent. Recent arguments have focused on what might be called the sadistic argument. This argument is fatally flawed because sport fishing is not a sadistic activity.

In reply to a chapter by Robert Ware on the need to include, rather than eliminate, institutions in theories of liberation, the author warns that liberation theory must walk on both social and psychological legs and then argues that Ware’s comparative analysis of institutions fails to lead analysis into crucial reflection on how individuals are transformed. Drawing on the work of John Dewey and George Benello, the author argues that an educational philosophy can offer a helpful framework for thinking about (...) relationships between institutions and individuals, such that genuinely democratic institutions would be recognized as being more developmentally educative for individuals involved. This essay will focus on one singular element: did the final ruling enhance or detract from our collective power to exercise stewardship over our health care resources? -/- Clearly Americans diverge on key (...) features of a desirable society and on the wisdom of using government to achieve even mutually desirable goals. But before politics settles the fate of the Affordable Care Act (and perhaps also the federal role in health policy for the foreseeable future), we should focus on what the Court has allowed us to consider: if we want it to, federal power may constitutionally be marshalled to compel insurers to end discrimination against the sick and to offer more transparent products so the marketplace will better serve consumers. One ofthe few completely nationalised systems of health carein the world, the NHS is seen by many as a moralbeacon of what it means to provide equitable medicaltreatment to all citizens on the basis of need andneed alone. However, others argue that it has failedto achieve the overall goals for which it was created.Because of scarce resources, some urgently needed careis not available at all, (...) while that which is receivedis sometimes second class. For these reasons, it isclaimed that the NHS should be scrapped and replacedby other systems of health care delivery.This paper outlines the history of the NHS,indicating some of the problems and innovations whichhave led to its current organization and structure.The philosophical foundations of the NHS are then articulated and defended on the grounds that it stillrepresents a morally coherent and economicallyefficient approach to the delivery of health care.Scarce resources are the key problem facing the NHS,making rationing inevitable and it is shown thatthis is not incompatible with the moral foundations ofthe service. The advent of simple in vitro fertilisation techniques has provided the reproductive biologist with an invaluable system for assaying sperm fertilising ability. In particular, they provide a useful way of identifying and characterising gamete‐specific proteins that play a role in sperm‐egg interactions, and in recent years, a growing number of sperm surface proteins have been identified that appear to be involved in these processes. Fertilin β was one of the first sperm membrane proteins to be implicated in egg interactions and (...) it has been proposed that this is mediated by means of binding of its disintegrin‐like domain to cognate integrin receptors on the egg plasma membrane. A recent paper in Science by Cho and colleagues [Cho et al. 1998. Fertilisation defects in sperm from mice lacking fertilin β. Science 281:1857–1859 (Ref. 1)] provides preliminary data on a fertilin β knockout mouse. Whilst fertilin βnull males had greatly reduced fertility, somewhat surprisingly, this could be largely attributed to causes other than impaired binding to the egg plasma membrane. In this paper I will argue that despite being uncertain about the existence of moral reasons, someone can still have a practical reason to act in a particular way. This practical reason is morally relevant because it will have an impact on whether we’re making the correct moral decision. This practical reason will result from a principle of decision-making that can be (...) used when someone is agnostic about the existence of moral reasons. The aims of this paper include explicitly beginning the discussion about this topic and advocating for a principle of moral decision-making that can be used despite being metaethically agnostic. Inspired by recent progress in multi-agent Reinforcement Learning (RL), in this work we examine the collective intelligent behaviour of theoretical universal agents by introducing a weighted mixture operation. Given a weighted set of agents, their weighted mixture is a new agent whose expected total reward in any environment is the corresponding weighted average of the original agents' expected total rewards in that environment. Thus, if RL agent intelligence is quantified in terms of performance across environments, the weighted mixture's intelligence is (...) the weighted average of the original agents' intelligences. This operation enables various interesting new theorems that shed light on the geometry of RL agent intelligence, namely: results about symmetries, convex agent-sets, and local extrema. We also show that any RL agent intelligence measure based on average performance across environments, subject to certain weak technical conditions, is identical (up to a constant factor) to performance within a single environment dependent on said intelligence measure. The question of what distinguishes moral problems from other problems is important to the study of the evolution and functioning of morality. Many researchers concerned with this topic have assumed, either implicitly or explicitly, that all moral problems are problems of cooperation. This assumption offers a response to the moral demarcation problem by identifying a necessary condition of moral problems. Characterizing moral problems as problems of cooperation is a popular response to this issue – especially among researchers empirically studying the (...) beginnings and limits of moral psychology. However, demarcating the moral in this way severely restricts the domain of moral problems. There are plenty of moral problems that aren’t simply problems of cooperation. In this paper I argue that understanding moral problems as problems of cooperation is too restrictive and offer an alternative way of demarcating moral from non-moral problems. Characterizing what makes a problem moral in terms of cooperation excludes a variety of problems that are ordinarily understood and responded to as moral. The alternative characterization that I propose is based on the American Indian/Native American concept of harmony. Using the concept of cooperation to demarcate the moral removes moral agents from their surroundings or contexts by assuming moral agency applies only to humans or other similarly evolved lifeforms. In contrast, using the concept of harmony allows for moral consideration to be granted to non-humans as well (e.g., non-human animals, plant life, ecosystems, etc.). (shrink)

Obesity is a particularly vexing public health challenge, since it not only underlies much disease and health spending but also largely stems from repeated personal behavioral choices. The newly enacted comprehensive health reform law contains a number of provisions to address obesity. For example, insurance companies are required to provide coverage for preventive-health services, which include obesity screening and nutritional counseling. In addition, employers will soon be able to offer premium discounts to workers who participate in wellness programs that emphasize (...) behavioral choices. These policies presume that government intervention to reduce obesity is necessary and justified. Some people, however, argue that individuals have a compelling interest to pursue their own health and happiness as they see fit, and therefore any government intervention in these areas is an unwarranted intrusion into privacy and one's freedom to eat, drink, and exercise as much or as little as one wants. This paper clarifies the overlapping individual, employer, and social interest in each person's health generally to avoid obesity and its myriad costs in particular. The paper also explores recent evidence on the impact of government interventions on obesity through case studies on food labeling and employer-based anti-obesity interventions. Our analysis suggests a positive role for government intervention to reduce and prevent obesity. At the same time, we discuss criteria that can be used to draw lines between government, employer, and individual responsibility for health, and to derive principles that should guide and limit government interventions on obesity as health reform's various elements (e.g., exchanges, insurance market reforms) are implemented in the coming years. The obesity epidemic is not only impairing the health of millions of Americans but also giving rise to billions of added dollars in health care spending. Climbing rates of obesity over the past decades are one of the predominant determinants behind the surging progression of health care expenses in the United States. Moreover, the less fit and less productive U.S. workforce has gradually eroded the nation’s industrial competitiveness. Since the early 1970s, adult obesity rates have doubled and childhood obesity rates (...) have more than tripled, while health expenditures have risen two percentage points faster than the Gross Domestic Product, burgeoning from 8.8 percent in 1980 to a projected 17.9 percent in 2011. Studies analyze that greater than a quarter of America’s health care expenses are attributed to obesity. The stunning growth in obesity has been imputed for 20 to 30 percent of the increase in health care costs since the late 1970s. If the proportion of obese population had stayed unchanged, then health care expenditures in America would be approximately 10 percent less on a per capita average than they are today. (shrink)

This article is an attempt to bring together knowledge about the Norwegian prostitution market, public debates on prostitution and prostitution laws and regulation in order to explore the processes whereby the prostitution setting is constituted. Norway has been the site of changes in the ways female prostitution takes place, changes that are being experienced by the women involved due to a growth in indoor prostitution. The study aimed to investigate the mental health problems and related factors among healthcare staff in Vietnam.MethodsA descriptive cross-sectional mixed methods study, combining quantitative and qualitative research methods, was performed among 400 healthcare workers working at the National Hospital for Tropical Diseases and Ninh Binh General Hospital from the first day of treatment for COVID-19 patients to May 01, 2020.ResultsThe results (...) showed that 8.0% of participants had stress, 17.5% of participants had anxiety, and 14.8% of participants had depression. Approximately 50% of participants reported that they had at least one of these symptoms. The findings illustrated that stress, anxiety, and depression were associated with the position in a hospital, health status during the COVID-19 pandemic, family members/relatives infected with COVID-19, physical and mental support from friends, family, and community, department, years of working, and the average work hours per day of healthcare workers exposed to COVID-19.ConclusionDuring the COVID-19 pandemic, healthcare workers who worked in the hospital providing treatment and care for COVID-19 patients dealt with mental health problems such as stress, anxiety, and depression. It is necessary to promote mental health among healthcare workers, to contribute to the fight against the COVID-19 outbreak in Vietnam. (shrink)

I propose a cautionary assessment of the recent debate concerning the impact of the dynamical approach on philosophical accounts of scientific explanation in the cognitive sciences and, particularly, the cognitive neurosciences. I criticize the dominant mechanistic philosophy of explanation, pointing out a number of its negative consequences: In particular, that it doesn’t do justice to the field’s diversity and stage of development, and that it fosters misguided interpretations of dynamical models’ contribution. In order to support these arguments, I analyze a (...) case study in computational neuroethology and show why it should not be understood through a mechanistic lens; I specially focus on Zednik’s mechanistic interpretation of the case study. In addition, I argue for a greater appreciation of the relation between explanation and other epistemic goals in the field, as well as an increased sensitivity towards the associated contextual factors. Sexual difference must be recognized in thought and human interactions to enable people to find their real source of jouissance. The Lacanian finding of ‘masculine phallic desire’ and ‘feminine libidinal desire’ in all human beings, irrespective of their biological sex-gender, paved the way for rethinking gender identities. It calls for reforming inter-human relations from the presently predominant utilitarian mode to the one based on love and recognition of the other. Gender is an arbitrary construct to serve the interests of males (...) in finding phallic sexual pleasure, power and recognition in their social life. Assertion of sexual difference and gender multiplicity will enable human beings to find the other human being as their actual desired object, beyond the pleasures offered by wealth, comforts and commodities.
